Before your furry friend's first visit to the Pooch Palace we will need the following forms filled out.

• Customer Information
• Agreement
• Permission to treat authorization

We will also need due dates for the following vaccines:
Rabies - Bordetella (aka Kennel Cough) - Distemper (DHPP)

You can either bring your dog's vet records or contact us with your vet's name in order for us to obtain the records prior to your visit.

Female dogs must be spayed and male dogs must be neutered in order to be accepted for daycare and/or boarding at the Pooch Palace.
